Please note that eGAP automatically defaults to FY2027 at the start the new Fiscal Year. If you are working in FY2026, you will need to change years before completing ES2's or revising applications.
FY 27 Title IV-A Program Evaluations

Please utilize the Program Evaluation (Previous Year) and Program Evaluation (Current Year) forms when completing Title IV-A documentation. These forms have been updated to incorporate the required goals and objectives directly into the forms.
Note: Previous versions of the forms are still listed. If you have already started using the previous forms, you may continue with those for your current submission.
